FT Case Manager- HUB, La Crosse

FT Community Health Worker needed for La Crosse HUB Program to serve as a Community Care Coordinator (CCC) that will work with individuals who have chronic disease or are pregnant.

Successful applicants will be able to provide social, cultural, behavioral, and resource supports to clients as they navigate the health care and social service systems.

Applicant must currently live or have previously lived in the City of La Crosse Census Tracts 2, 3, 4, or 5 (https://censusreporter.org/)

  • Minimum of one-year experience working or volunteering in human services, healthcare, community outreach, health education, or related direct service
  • Knowledge of local resource agencies and resources available
  • Some college preferred.

PT Direct Care Staff- Day Treatment, Winona

Responsibilities include:

  • Supervision of youth
  • Transportation
  • Providing crisis intervention

Requirements:

  • Must be 20 years of age
  • Have a good driving record
  • Experience and/or educational background is preferred

 

PT Direct Care Staff- The Other Door, Viroqua

The Other Door in Viroqua is looking for  Direct Care Staff to cover evening shifts.

Responsibilities include:

  • Provide programming to promote socialization
  • Provide attendees with information about recovery and mental health resources

Requirements:

  • Preference given to candidates who have experienced their own recovery from mental illness or substance abuse
  • Work or volunteer experience providing peer support preferred
  • Current certification or willingness to complete training and course certification is encouraged
