Healthy Families in Winona is a comprehensive and intensive home visitation program that has a full-time opening for a Case Manager to work with a supportive team and establish a trusting relationship with overburdened families by delivering service that is culturally competent, strength-based, family centered and family directed.

Responsibilities Include:

  • Meet with families in the least restrictive environment, typically their homes or in the community in Winona County
  • Assist parents in improving their skills so as to optimize a positive home environment
  • Increase the family’s ability to problem-solve and advocate for themselves and their children
  • Identify and refer families to additional community resources, including healthcare and other basic necessities
  • Work cooperatively with families to establish goals and develop a plan for accomplishing those goals
  • Conduct regular assessments of the growth and development of the children participating in the Healthy Families program
  • Suggest, lead, and model parent/child activities
  • Maintain written documentation of all activities and contact with families, as well as referrals or contacts made on behalf of families
  • Participates in all agency staff meetings, team meetings, and other meetings as assigned

Requirements:

  • Minimum of a bachelor’s degree in human services or related field preferred
  • Working knowledge of prenatal and child development, mental health and community resources in order to provide support to young families
  • Access to personal vehicle
  • Valid driver’s license and vehicle insurance
  • Good driving record

 

 

 

Independent Living in La Crosse is hiring for a PT Independent Living Specialist with the Independent Living Program! This position will work approximately 20 hours per week.

Young adults, ages 18-21, who have aged of foster care, are eligible for Independent Living Services. As an Independent Living Specialist, you will focus on resource and skill development to help young adults live successfully in the community by building an Individualized Life Skills Plan.

Responsibilities Include:

  • Case Management of youth
  • Planning and facilitating groups
  • Providing one-on-one services with the youth, including assistance with:
    • College applications
    • Job searches
    • Budget management
    • Obtaining affordable housing
    • Meal planning
  • Attending team meetings.

Requirements:

  • Minimum of a bachelor’s degree in human services or related field preferred
