PT or FT Therapist in Outpatient Services, Winona

Outpatient Services in Winona, MN is hiring 2 PT and/or FT Therapists. Applicants must have a Master’s or Doctorate degree in one of the following areas: LPCC, MFT, LICSW, LPP, PNP, or Psychiatrist and be licensed and third-party billable as a mental health professional.

Essential Duties include:

  1. Direct Care Services: Provide psychotherapy services within scope of expertise to individuals, couples, family, and/or groups. Services include but are not limited to: individual therapy; group therapy; family therapy; in-home therapy; diagnostic assessments, other assessments or direct care services as trained or qualified. Maintain full caseload, equal to no less than 70% of time, allocated for psychotherapy and or other billable direct services. Professionals or clinical trainees with special interest in working with children ages birth to five encouraged to apply.
  2. Documentation: Maintain client files, ensuring completed and timely documentation of provided services in accordance with agency policies and governing standards. Complete all required forms and process in the electronic health records (EHR) system in a timeframe consistent with service and billing requirements.
  3. Training & Consultation: Complete or maintain training on best practice and evidence based models with successful outcomes. Stay abreast of new developments in clinical area of expertise. Communicate and share new knowledge with other clinical staff during clinical consultation. Seek out appropriate consultation with supervisor or other clinicians as needed when encountering challenging cases. Maintain certification/licensure in good standing, and remain current in required hours of annual staff development.
  4. Agency Participation: Participate in scheduled individual and/or group supervision, staff meetings, and other agency meetings, committees, or groups as requested. Perform community presentations or other outreach services as required.

 

FT Case Manager, Healthy Families, Winona

The Healthy Families program of Family and Children’s Center is a comprehensive and intensive home visitation program that has a full-time opening for a Case Manager to work with a supportive team and establish a trusting relationship with overburdened families by delivering service that is culturally competent, strength-based, family centered and family directed.

Responsibilities Include:

• Meet with families in the least restrictive environment, typically their homes or in the community

• Assist parents in improving their skills so as to optimize a positive home environment

• Increase the family’s ability to problem-solve and advocate for themselves and their children

• Identify and refer families to additional community resources, including healthcare and other basic necessities

• Work cooperatively with families to establish goals and develop a plan for accomplishing those goals

• Conduct regular assessments of the growth and development of the children participating in the Healthy Families program

• Suggest, lead, and model parent/child activities

• Maintain written documentation of all activities and contact with families, as well as referrals or contacts made on behalf of families

• Participates in all agency staff meetings, team meetings, and other meetings as assigned

Requirements:

  • Minimum of a bachelor’s degree in human services or related field
  • 3 years experience after bachelor’s degree including case management experience
  • Working knowledge of prenatal and child development, mental health and community resources in order to provide support to young families
  • Access to personal vehicle
  • Valid driver’s license and vehicle insurance
  • Good driving record

 

 

FT Case Manager, CSP, Viroqua

CSP Vioqua is seeking a FT Case Manager. Bachelor’s degree required, Master’s degree preferred.

Responsibilities include:

  • Providing comprehensive mental health services to adults with severe and persistent mental illnesses, including but not limited to:
    • Assessment, treatment, and discharge planning
    • Implementation of services as needed to assist the client in meeting their recovery plan goal
    • Assisting the client in managing psychiatric symptoms, medical issues that co-occur with mental health issues and substance use issues.
    • Ensuring the client’s basic needs are met
    • Attending all scheduled treatment meetings
  • Completion of all client service related paperwork
  • Provide case management services as necessary
  • Provide after-hours crisis services to CSP clients by phone or as needed face to face

 

Independent Living Specialist, IL La Crosse

INDEPENDENT LIVING SPECIALIST needed to work on independent living skills with youth that are in or have been in out-of-home-care in a 15 county region. Bachelor’s degree required.

Responsibilities include conducting assessments, groups, and one-on-one services with the youth, as well as attending team meetings. The Independent Living Specialist works closely with and supervises interns.

This position will cover the following 7 counties: Trempealeau, La Crosse, Jackson, Monroe, Juneau, Vernon and Crawford

While this position reports to the supervisor in our La Crosse office, this is a semi-remote position in that it provides in person services to clients in the above counties.
